本人，              授權及同意中國教育部留學(xué)服務(wù)中心可就我本人學(xué)歷學(xué)位認證的有關(guān)事宜向相關(guān)機構或組織透露本人的個(gè)人資料并對本人取得國（境）外學(xué)歷學(xué)位證書(shū)及高等教育文憑的情況進(jìn)行核查。同時(shí)也授權相關(guān)組織或機構可就這些核查內容，向中國教育部留學(xué)服務(wù)中心透露任何有關(guān)的記錄及資料。

Declaration

I，            authorize and consent to the Chinese Service Center for Scholarly Exchange （CSCSE）making any enquiries, which requires releasing my personal information, for purposes relating to qualification evaluation. I also authorize and consent to relevant organizations/schools to release my record or information to the CSCSE as may be required for these enquiries.
